Step Two - Complete Course Verification Form
Following completion of this course, please click here to complete a required course verification form. The information you provide on the form is necessary in order to obtain your CE certificate from the VDHA Continuing Education Committee. Upon completing the form, please allow 14 days for processing your CE certificate which will be emailed to you. This CE certificate should be kept permanently in your files, as it validates your compliance with Board regulations for remote supervision practice.
